This still works, but I think this form of address isn't supported:
http://cache.googlevideo.com/get_video?video_id=<id>&origin=lax-v289.l
ax.youtube.com

Meaning that the end starting from &origin should be removed, the
youtube-address replaced as host and then given as a wget argument. Or is
everyone content doing this manually?

Watch out for videos with ':' in the name. Results in illegal filename.

I suppose mplayer has to retrieve the full video file before it will start
playing it?
